As a Primary Care Physician (MD/DO), you will work in an outpatient clinic setting, managing a small patient panel to ensure personalized attention and continuity of care. You will deliver comprehensive primary care services to seniors, many of whom have multiple chronic conditions. The clinic provides robust support, including dedicated medical assistants, schedulers, and referral specialists, so you can focus on what matters most: delivering exceptional care and fostering long-term relationships with your patients. Primary Care Physician (MD/DO) – Summary- Full-time outpatient position in a collaborative, mission-driven environment

- Manage a panel of up to 450 senior patients, providing comprehensive primary care services
- Conduct thorough patient assessments and develop individualized treatment plans
- Monitor and manage chronic conditions in a value-based care model
- Work closely with a dedicated support team, including medical assistants, schedulers, and referral specialists
- Collaborate with in-house specialists such as cardiologists and podiatrists for integrated patient care
- Document patient encounters and coordinate care within the clinic’s electronic medical record system
- Participate in holistic health offerings and wellness initiatives as part of a comprehensive approach to senior care
- Uphold high standards of patient safety, quality, and satisfaction
- Engage in ongoing professional development and participate in clinic meetings and quality improvement initiatives

- Completion of residency training in Internal Medicine, Family Medicine, or Geriatrics
