Primary Care Physician (MD/DO) - Summary
- Full-time outpatient role focused on providing comprehensive primary care to adult and senior populations.

Duties & Responsibilities
- Provide primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions.
- Manage a daily patient schedule averaging 18 patients per day, Monday through Friday, within clinic hours of 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm).
- Deliver 100% outpatient care; no night or weekend shifts required.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist.
- Participate in value-based care initiatives to improve patient outcomes and satisfaction.
- Utilize in-house consulting specialists for cardiology, podiatry, and other specialties as needed.
- Engage patients in holistic health programs such as acupuncture, tai chi, yoga, and nutrition services offered by the clinic.
- Ensure accurate documentation and administrative tasks are completed during scheduled administrative time.
- Advocate for patients, coordinate care, and provide education on managing chronic conditions.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered care to diverse populations.
- Strong clinical skills in managing chronic diseases and preventive care for older adults.
- Excellent communication, interpersonal, and teamwork abilities.
- Eligibility for medical licensure.
