Primary Care Physician (MD/DO) - Summary
- Full-time position in a physician-led, value-based primary care organization
- Focus on delivering comprehensive outpatient care to seniors
- Work with a small, manageable patient panel for optimal patient relationships
- Collaborate with a dedicated care team for support and efficiency
Duties & Responsibilities
- Provide high-quality, compassionate primary care services in an outpatient setting
- Manage a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct patient visits, typically 18 per day, ensuring thorough evaluation and follow-up
- Work Monday to Friday, with clinic hours from 7:30 am to 5:00 pm
- Participate in value-based care initiatives and quality improvement programs
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) for comprehensive care
- Engage in holistic health services, including supporting patient participation in acupuncture, Tai Chi, yoga, and nutrition counseling
- Utilize provided administrative time for care coordination and documentation
- Benefit from robust support staff, including a dedicated medical assistant, scheduler, and referral specialist
- Ensure compliance with all clinical policies, procedures, and regulatory requirements
Salary & Benefits
- Competitive base salary (starting at $250,000+ annually)

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board certification or board eligibility in FM, IM, or Geriatric Medicine

- Excellent communication, collaboration, and organizational skills
