Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Conduct comprehensive primary care for adult and geriatric patients, including diagnosis, treatment, and ongoing management of chronic conditions.
- Maintain a daily patient schedule of approximately 18 patients, ensuring personalized attention for each visit.
- Collaborate closely with a care team, including a medical assistant, scheduler, and referral specialist, to coordinate holistic patient care.
- Utilize value-based care models focused on improving patient outcomes rather than patient volume.
- Engage in interdisciplinary case reviews with in-house consulting specialists such as cardiology and podiatry.
- Document patient encounters, treatment plans, and progress accurately and efficiently within the electronic medical record system.
- Participate in and contribute to practice-wide quality improvement initiatives and clinical meetings.
- Ensure adherence to all applicable clinical guidelines, standards, and regulatory requirements.
- Promote holistic health services, including access to acupuncture, tai chi, yoga, and nutrition counseling.
- Support patient access with transportation services and administrative time included in your schedule.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to value-based care and population health management principles.
- Strong interpersonal and communication skills, with a patient-centered approach to care.
