As a Primary Care Physician (MD/DO) in our group, you will manage a small patient panel of up to 450 patients, allowing you to build meaningful relationships and practice true patient-centered medicine. You will care for a patient population with complex needs, primarily seniors with multiple chronic conditions, utilizing a value-based care model. Our practice supports ongoing professional development and offers a clear pathway to partnership, with the majority of physicians achieving partner status within 12 to 18 months. Partnership requires no buy-in, reflecting our commitment to collaborative leadership and shared success.

Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Manage a panel of up to 450 patients, primarily seniors with 3-5 chronic conditions
- Provide comprehensive outpatient primary care, focusing on prevention, chronic disease management, and overall wellness
- See an average of 18 patients per day in a Monday through Friday clinic schedule
- Collaborate with an in-house team of consulting specialists, including cardiology and podiatry
- Utilize holistic health resources such as acupuncture, tai chi, yoga, and nutrition counseling to support patient wellness
- Leverage a dedicated care team for administrative support, care coordination, and referral management
- Participate in value-based care initiatives to improve patient outcomes and drive quality improvement
- Include administrative time in your schedule to ensure thorough documentation and care planning
- No nights or weekends; 100% outpatient responsibilities

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Completion of an accredited residency in Internal Medicine or Family Medicine
- Commitment to value-based, patient-centered care and continuous quality improvement
- Excellent communication, organizational, and teamwork skills
- Dedication to serving and improving the health of the senior population
