Key Responsibilities
- Deliver comprehensive, patient-centered primary care to a senior patient panel, focusing on the management and treatment of multiple chronic diseases.
- Manage a panel of up to 450 patients, ensuring individualized attention and continuity of care.
- Conduct 100% outpatient care with no night or weekend shifts required, maintaining a healthy work/life balance.
- See an average of 18 patients per day, allowing for thorough and effective care delivery.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to support optimal patient outcomes.
- Participate in value-based care initiatives and follow best practices for improved health outcomes among senior patients.
- Utilize administrative time built into your schedule to complete necessary documentation and care coordination.
- Engage with in-house consulting specialists, including cardiology and podiatry, to provide holistic care.
- Leverage clinic resources such as holistic health services (acupuncture, tai chi, yoga, and nutrition) tailored to senior care.
- Ensure patients have access to free door-to-door transportation for clinic visits, enhancing accessibility and adherence to care plans.
Qualifications
- Medical degree (MD or DO) and completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to patient-centered, value-based care for seniors.
- Excellent communication and collaboration skills within a multidisciplinary team environment.
- Ability to manage complex chronic conditions and coordinate comprehensive care plans.
