Primary Care Physician (MD/DO) – Summary
- Full-time role for a board-certified or board-eligible physician (MD/DO)
- Focus on serving senior patients with chronic illnesses

Duties & Responsibilities
- Manage a patient panel of up to 450 seniors, with an average of 18 patients per day
- Deliver 100% outpatient primary care services with a value-based approach
- Utilize administrative time built into the schedule for documentation and care coordination
- Work collaboratively with a designated care team, including a medical assistant, scheduler, and referral specialist
- Participate in partnership opportunities (typically available within 12-18 months, with no buy-in required)
- Engage with in-house consulting specialists such as cardiology, podiatry, and other disciplines
- Promote holistic health services, including acupuncture, Tai Chi, yoga, and nutritional counseling
- Ensure continuity of care and foster long-term patient relationships
- Contribute to the achievement of organizational quality and patient satisfaction goals

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Completion of residency training in Internal Medicine or Family Medicine
- Board certification or board eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to value-based care and senior health
- Strong communication, organizational, and interpersonal skills
