Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, focusing on value-based care models
- Manage and treat patients with 3-5 chronic conditions, developing individualized care plans
- Conduct patient visits in a 100% outpatient setting, averaging 18 patients per day
- Coordinate with dedicated support staff, including medical assistants, schedulers, and referral specialists
- Collaborate with in-house specialists in cardiology, podiatry, and other disciplines to ensure holistic patient care
- Utilize integrated health services such as acupuncture, Tai Chi, yoga, and nutrition counseling to support patient wellness
- Participate in a Monday through Friday schedule, with administrative time built into your workday
- Maintain thorough documentation and comply with all relevant regulations and best practices
Salary & Benefits
- Competitive base salary starting at $250,000, with additional performance-based incentives
- Signing bonus potential up to $50,000
- Eligibility for partnership within 12-18 months, with no buy-in required

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Completion of an accredited residency in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Demonstrated commitment to value-based, patient-centered care
- Spanish language proficiency is preferred but not required
- Excellent communication, teamwork, and organizational skills
