Primary Care Physician (MD/DO) Job Description
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time opportunity in Philadelphia, PA. Responsibilities
- Deliver comprehensive primary care services to an assigned panel of patients, focusing on seniors with multiple chronic conditions.
- Manage a patient panel with a maximum of 450 patients, allowing for personalized and attentive care.
- Conduct 100% outpatient care with no night or weekend shifts required, supporting excellent work/life balance.
- Provide care to an average of 18 patients per day, ensuring adequate time for each patient interaction.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist.
- Utilize value-based care models to improve patient health outcomes and reduce hospitalizations.
- Participate in regular administrative time built into your schedule for documentation and coordination of care.
- Refer patients to in-house consulting specialists in areas such as cardiology and podiatry as appropriate.
- Support patients’ access to holistic health services, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure that patients who require it have access to complimentary door-to-door transportation services for clinic visits.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered care to a senior population.
- Excellent communication and teamwork skills.
- Spanish language proficiency is preferred but not required.
