As a Primary Care Physician (MD/DO), you will manage a small patient panel, ensuring personalized attention and continuity of care. You will have access to in-house consulting specialists, holistic health services, and a dedicated care team, including your own medical assistant, scheduler, and referral specialist. Primary Care Physician (MD/DO) – Summary
- Full-time outpatient role delivering primary care to adult and senior patients.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, primarily seniors with multiple chronic conditions.
- Conduct patient visits (approximately 18 per day), focusing on preventative care, chronic disease management, and wellness.
- Collaborate with a dedicated care team including a medical assistant, scheduler, and referral specialist to coordinate patient care efficiently.
- Utilize in-house consulting specialists for cardiology, podiatry, and other disciplines as needed to provide holistic patient care.
- Incorporate holistic health services such as acupuncture, Tai Chi, yoga, and nutritional counseling into care plans where appropriate.
- Ensure effective communication and partnership with patients and their families, particularly in the management of complex health needs.
- Support patient access with door-to-door transportation services provided at no cost to patients.
- Practice within a value-based care model, focusing on quality outcomes and patient satisfaction.
- Participate in regular administrative time, allowing for thorough documentation, care planning, and professional development.
- Maintain compliance with all organizational policies and relevant healthcare regulations.
Salary & Benefits

Qualifications & Requirements
- Medical degree (MD or DO) and completion of accredited residency training in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated experience or interest in value-based primary care and management of complex, chronic conditions in older adults.
- Spanish language proficiency is preferred but not required.
- Commitment to delivering high-quality, patient-centered care within a collaborative team environment.
