Primary Care Physician (MD/DO) – Philadelphia, Full-time Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a physician-led, mission-driven healthcare organization in Philadelphia. This full-time opportunity focuses on delivering high-quality, value-based care to a senior patient population. As a Primary Care Physician, you will play a pivotal role in improving patient health outcomes while working with a supportive clinical team in a collaborative and innovative environment.
Responsibilities
- Provide comprehensive outpatient primary care to a patient panel primarily consisting of seniors with multiple chronic conditions.
- Manage a maximum of 18 patients per day, ensuring adequate time for each patient encounter and fostering strong provider-patient relationships.
- Participate in a Monday through Friday schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. and no required nights or weekends, supporting work-life balance.
- Utilize a value-based care model to improve patient outcomes and enhance the overall patient experience.
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to coordinate and optimize patient care.
- Engage in administrative duties during built-in schedule time, allowing for effective patient management and thorough documentation.
- Refer patients to in-house consulting specialists such as cardiology and podiatry, and support holistic health initiatives including acupuncture, tai chi, yoga, and nutrition.
- Embrace the mission and vision of the organization by participating in continuous quality improvement and clinical leadership initiatives.
Qualifications
- Medical degree (MD or DO) with completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing compassionate, high-quality care to a diverse senior patient population.
- Spanish language proficiency is preferred but not required.
- Strong interpersonal communication and teamwork skills.
