Primary Care Physician (MD/DO) – Summary
- Deliver outpatient primary care to a senior patient population with multiple chronic diseases.
- Work with a small patient panel to promote individualized care and stronger patient-provider relationships.

Duties & Responsibilities
- Provide high-quality, patient-centered primary care services to a panel of adult and senior patients, focusing on preventive care and chronic disease management.
- Conduct comprehensive assessments, develop care plans, and coordinate ongoing medical care for patients, primarily seniors with 3-5 chronic conditions.
- Work closely with a dedicated care team, including medical assistants, schedulers, and referral specialists, to optimize patient outcomes and experience.
- Utilize value-based care principles to achieve better health outcomes and patient satisfaction.
- Maintain a daily patient load of approximately 18 patients, ensuring quality time and attention for each individual.
- Participate in administrative activities such as charting, care coordination, and quality improvement initiatives, with administrative time built into the schedule.
- Collaborate with in-house consulting specialists in cardiology, podiatry, and other disciplines as needed.
- Engage in holistic health initiatives, including access to acupuncture, Tai Chi, yoga, and nutritional counseling for patients.
- Adhere to Monday through Friday clinic hours (7:30 a.m. to 5:00 p.m.), with the last patient scheduled at 4:15 p.m. No nights or weekends required.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered care to a senior population.
- Experience with or strong interest in value-based care models.
- Proficiency in Spanish is preferred, but not required.
