Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient primary care role serving a senior population
- Focus on value-based care and preventative medicine

Duties & Responsibilities
- Provide comprehensive primary care to a maximum panel of 450 patients, primarily seniors with multiple chronic conditions
- See an average of 18 patients per day in a 100% outpatient setting
- Conduct thorough patient assessments, develop individualized care plans, and manage ongoing treatment for chronic diseases
- Participate in value-based care initiatives to improve patient health outcomes
- Work closely with an assigned support team, including a medical assistant, scheduler, and referral specialist
- Utilize in-house consulting specialists, such as cardiology and podiatry, to provide coordinated care
- Embrace holistic health services offered at the clinic, including acupuncture, Tai Chi, yoga, and nutrition
- Allocate dedicated administrative time within your schedule for optimal workflow
- Maintain work/life balance with a Monday through Friday schedule, no evening or weekend hours
- Ensure all care is compliant with organizational protocols, industry standards, and regulatory requirements

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Commitment to delivering high-quality, patient-centered care
- Spanish language proficiency preferred but not required
- Demonstrated ability to work effectively within a multidisciplinary team
- Excellent communication, organizational, and problem-solving skills
