Primary Care Physician (MD/DO) – Philadelphia, PA – Full-Time Overview
We are seeking a dedicated and mission-driven Primary Care Physician (MD/DO) to join our physician-led organization in Philadelphia, PA. Our team is focused on delivering affordable, high-quality primary care to seniors, with a commitment to transforming healthcare for those who need it most. As a full-time Primary Care Physician, you will play a vital role in providing comprehensive outpatient care while working alongside a supportive clinical team. Working in Philadelphia, PA offers the opportunity to serve a diverse community and make a significant impact on the health and wellbeing of local seniors.
Key Responsibilities
- Provide comprehensive primary care services to a panel of adult and senior patients, focusing on those with multiple chronic conditions.
- Manage a patient panel with a maximum of 450 individuals, ensuring personalized and attentive care.
- See approximately 18 patients per day in a 100% outpatient setting.
- Collaborate with an integrated care team, including a dedicated Medical Assistant, Scheduler, and Referral Specialist.
- Utilize value-based care principles to improve patient outcomes and deliver holistic health services.
- Participate in clinic activities Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
- Ensure work-life balance through scheduled administrative time and no required nights or weekends.
- Refer patients to in-house consulting specialists such as Cardiology and Podiatry, and leverage holistic health resources including acupuncture, tai chi, yoga, and nutrition services.
- Support senior patients, many of whom have 3-5 chronic diseases, and coordinate care to address complex health needs.
Qualifications
- Medical Degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to excellence in patient care and a mission-driven approach to healthcare delivery.
- Spanish language proficiency is preferred but not required.
