As a Primary Care Physician (MD/DO), you will have the opportunity to work alongside experienced clinical leaders and a dedicated care team. This role is perfect for driven, compassionate professionals who are board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine. You will manage a small, focused patient panel, allowing you to spend more time with each patient and truly impact their health outcomes. Administrative support and resources are provided, so you can focus on patient care without the burden of excessive administrative tasks.
Our clinics offer a comprehensive suite of in-house consulting services, including cardiology and podiatry, as well as holistic health services like acupuncture, tai chi, yoga, and nutrition counseling. These resources allow you to address the full spectrum of your patients’ health needs. In addition, transportation is provided to patients at no charge, ensuring access to care for those who need it most.
This is a full-time, 100% outpatient opportunity with a Monday through Friday schedule, no nights or weekends, and a strong emphasis on work/life balance. You will have the chance to pursue partnership within 12–18 months, with the majority of physicians achieving this milestone in their first year—without any buy-in requirement. Primary Care Physician (MD/DO) - Summary- Physician-led, mission-driven primary care organization serving seniors in Philadelphia, PA
- Focus on value-based, holistic care for a diverse and medically complex senior population
- Full-time, outpatient role with a Monday-Friday schedule
- Provide comprehensive primary care to a panel of up to 450 senior patients
- Conduct patient visits (approximately 18 patients per day) and develop personalized care plans
- Manage chronic disease and complex medical conditions, emphasizing preventative care
- Collaborate with a dedicated team, including your own Medical Assistant, Scheduler, and Referral Specialist
- Leverage in-house consulting specialists and holistic services to address patient needs
- Participate in value-based care initiatives and outcomes measurement
- Allocate time for administrative duties within scheduled work hours
- Maintain a high standard of documentation and compliance with regulatory guidelines

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Completion of an accredited residency in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Spanish language proficiency preferred, but not required
