Primary Care Physician (MD/DO) – Philadelphia, PA – Full-time Overview

As a Primary Care Physician (MD/DO) in our organization, you will have the opportunity to deliver patient-centered care while working with a multidisciplinary team that supports both you and your patients. Responsibilities
- Provide comprehensive, outpatient primary care services to a dedicated panel of senior patients, managing an average of 18 patients per day.
- Deliver care to a patient population with multiple chronic conditions, utilizing value-based care principles to improve health outcomes.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure coordinated and efficient patient care.
- Engage in regular communication with in-house consulting specialists in cardiology, podiatry, and other relevant fields to support holistic patient care.
- Participate in multidisciplinary approaches to health, including access to holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Maintain thorough and accurate patient records, dedicating built-in administrative time to documentation and care planning.
- Utilize the latest evidence-based practices to provide preventive care, chronic disease management, and acute care for seniors.
- Foster a welcoming and inclusive environment for all patients, upholding the organization’s mission to honor seniors by delivering better health outcomes.
Qualifications
- Medical degree (MD or DO) with completion of a residency in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to practicing evidence-based, value-driven primary care.
- Strong communication and interpersonal skills to build rapport with patients and colleagues.
- Spanish language proficiency is preferred but not required.
- Comfort working with a senior population managing multiple chronic diseases.

Work Schedule
- Full-time, outpatient role with a Monday through Friday schedule.
- Clinic hours are from 7:30 AM to 5:00 PM, with the last patient scheduled at 4:15 PM.
