Primary Care Physician (MD/DO) About the Opportunity
We are seeking a dedicated Primary Care Physician (MD/DO) to join a physician-led, mission-driven organization in Philadelphia, PA. This is a full-time position focused on delivering comprehensive outpatient care to a senior patient population. Key Responsibilities
- Provide direct primary care services to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct patient visits and follow-ups, averaging approximately 18 patients per day in an outpatient clinic setting.
- Work collaboratively within a multidisciplinary care team, including a dedicated medical assistant, scheduler, and referral specialist, to deliver patient-centered care.
- Emphasize value-based care principles to improve patient outcomes and satisfaction.
- Participate in care planning, health education, and chronic disease management tailored to the needs of each patient.
- Maintain accurate and timely documentation of patient encounters and care plans using electronic health records.
- Utilize administrative time embedded within the schedule to coordinate care and complete necessary documentation.
- No night or weekend shifts; clinic hours are Monday through Friday, 7:30 am to 5:00 pm, with the last patient scheduled at 4:15 pm.
- Participate in partnership opportunities within the organization, with many providers achieving partner status within 12–18 months.
Qualifications
- Medical degree (MD or DO) required.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Experience or strong interest in value-based primary care and chronic disease management for seniors.
- Commitment to providing inclusive and culturally competent care. Spanish language proficiency is preferred but not required.
