Primary Care Physician (MD/DO) - Summary- Full-time, 100% outpatient primary care position focused on adult and senior populations
- Mission-driven organization dedicated to transforming healthcare for those most in need
- Small patient panel to foster quality care and long-term relationships
- Supportive, team-based environment with dedicated administrative personnel
- Provide high-quality, compassionate care to a panel of adult and senior patients, primarily those with 3-5 chronic diseases
- Manage a maximum of 18 patients per day to ensure personalized attention and thorough care
- Conduct comprehensive medical assessments, diagnosis, and management for acute and chronic conditions
- Utilize a value-based care approach to improve patient outcomes and quality of life
- Work collaboratively with a dedicated care team, including your own Medical Assistant, Scheduler, and Referral Specialist
- Participate in holistic health initiatives such as nutrition, acupuncture, tai chi, and yoga offerings
- Leverage in-house consulting specialists, such as cardiology and podiatry, to optimize patient care
- Engage in regular administrative time built into your schedule, reducing after-hours work
- Work Monday to Friday, with no nights or weekends required
- Support patients’ access to care, including those requiring provided transportation services
- Competitive base salary (starting at $250,000) with performance-based incentive bonuses
- Potential for partnership, typically achievable within 12-18 months with no buy-in requirement
- Signing bonus available for qualified candidates

- Paid time off totaling 33 days annually
- Annual continuing medical education (CME) allowance and dedicated CME time
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Commitment to providing compassionate, patient-centered care
- Spanish language proficiency is preferred, but not required
- Passion for serving senior and vulnerable populations in a team-based primary care setting
- Experience in value-based care and managing patients with chronic conditions
- Collaborative, communicative, and adaptable to a holistic approach to medicine
