Primary Care Physician (MD/DO) – Summary
- Physician-led, mission-driven practice focused on senior care
- Full-time outpatient position in Richmond, VA
- Small patient panel (up to 450 patients)
- Collaborative, supportive care team model

Duties & Responsibilities
- Provide comprehensive primary care to a senior patient population, managing 3-5 chronic diseases per patient
- Oversee a maximum patient panel of 450, allowing for personalized, attentive care
- See approximately 18 patients per day during clinic hours (Monday to Friday, 7:30 a.m. – 5:00 p.m.; last patient scheduled at 4:15 p.m.)
- Utilize dedicated administrative time within your schedule to complete documentation and care coordination
- Collaborate closely with your assigned medical assistant, scheduler, and referral specialist
- Leverage in-house consulting specialists (e.g., Cardiology, Podiatry) and holistic health resources (e.g., Acupuncture, Tai Chi, Yoga, Nutrition)
- Deliver 100% outpatient care with no nights or weekends required
- Participate in value-based care initiatives to improve patient outcomes

Qualifications & Requirements
- MD or DO degree from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Experience or strong interest in caring for seniors with chronic conditions
- Commitment to value-based, patient-centered care principles
