Our patient population primarily consists of seniors managing multiple chronic conditions. The Primary Care Physician (MD/DO) will work with a maximum panel of 450 patients, supported by a dedicated care team that includes a Medical Assistant, scheduler, and referral specialist. This structure ensures that you have the time and resources needed to provide thorough, individualized care to each patient. With no nights or weekends required and administrative time built into your schedule, you can achieve a healthy balance between career and personal life.

Primary Care Physician (MD/DO) – Summary- Full-time Primary Care Physician role centered on senior patient care
- Physician-led, mission-driven organization focused on value-based care

- Manage a patient panel of up to 450 seniors, typically managing 3-5 chronic diseases
- Conduct comprehensive primary care visits (approximately 18 patients per day)
- Deliver value-based care focused on quality outcomes
- Utilize dedicated clinical support staff (MA, scheduler, referral specialist)
- Participate in a Monday–Friday schedule with built-in administrative time
- Collaborate with in-house consulting specialists and holistic health providers
- Provide 100% outpatient care with no night or weekend shifts required
- Leverage clinic resources for patient transportation and holistic services

- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Completion of an accredited residency program in Internal Medicine or Family Medicine
