Primary Care Physician (MD/DO) - Summary- Provide comprehensive primary care to seniors in an outpatient clinic setting.

- Manage a small patient panel (up to 450 patients) primarily consisting of seniors with multiple chronic conditions.
- Conduct patient assessments, develop treatment plans, and coordinate care with a dedicated support team (MA, scheduler, referral specialist).
- Deliver outpatient care exclusively; no inpatient, night, or weekend shifts.
- Utilize value-based care principles to improve patient outcomes and satisfaction.
- Collaborate with in-house consulting specialists and leverage holistic health services provided by the clinic.
- Participate in ongoing quality improvement initiatives within the clinic.

- Board certified or board eligible in FM, IM, or Geriatric Medicine.
