Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven primary care organization in Richmond, VA. This full-time role is designed for clinical professionals who are passionate about providing exceptional healthcare and improving the lives of seniors in the Richmond community. As a Primary Care Physician, you will play a pivotal role in transforming patient outcomes through a value-based care model that emphasizes holistic, patient-centered service.
Responsibilities
- Provide comprehensive outpatient primary care services to a dedicated panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct thorough patient evaluations, develop individualized care plans, and manage ongoing treatment for patients with 3-5 chronic diseases.
- Maintain a manageable schedule, seeing approximately 18 patients per day during clinic hours of Monday through Friday, 7:30 am to 5:00 pm.
- Utilize administrative time included in your schedule to ensure accurate documentation and coordinated care.
- Work collaboratively with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist to ensure optimal patient support and continuity of care.
- Engage in a value-based care delivery model, prioritizing quality outcomes and preventive care initiatives.
- Participate in holistic health initiatives, benefiting from onsite consulting specialists in cardiology, podiatry, and more, as well as services such as acupuncture, Tai Chi, yoga, and nutritional counseling.
- Ensure the highest standards of patient safety and satisfaction while adhering to organizational policies and regulatory requirements.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing compassionate, high-quality care to a diverse population of seniors.
- Excellent communication, interpersonal, and organizational skills.
- Ability to thrive in a collaborative, team-based environment focused on continuous improvement.
