Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive, patient-centered primary care services to a senior patient population.
- Collaborate with a multidisciplinary care team to provide holistic, value-based care.

Duties & Responsibilities
- Manage a small patient panel (up to 450 patients), averaging 18 patient visits per day.
- Conduct thorough assessments, develop individualized care plans, and provide ongoing management of chronic and acute conditions.
- Work Monday through Friday during clinic hours of 7:30 am to 5:00 pm, with the last patient scheduled at 4:15 pm.
- Dedicate time in your schedule for administrative responsibilities.
- Participate actively in value-based care initiatives focused on quality, outcomes, and patient satisfaction.
- Collaborate with on-site consulting specialists in cardiology, podiatry, and other fields as needed.
- Refer to and coordinate with holistic health services, including acupuncture, tai chi, yoga, and nutritional counseling.
- Serve as the primary point of contact for a senior patient population with multiple chronic diseases.
- Work exclusively in an outpatient setting with no night or weekend hours required.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to providing value-based, compassionate care to seniors living with multiple chronic conditions.
- Strong communication, clinical, and organizational skills.
