Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven healthcare organization in Richmond, VA. As a Primary Care Physician, you will play a pivotal role in transforming healthcare for the community’s neediest populations in Richmond, VA, ensuring that each patient receives attentive and coordinated care in a supportive environment.
Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions.
- Conduct thorough patient assessments, develop individualized care plans, and manage ongoing treatment for chronic diseases within an outpatient clinic setting in Richmond, VA.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure seamless patient support and care coordination.
- Utilize value-based care approaches to enhance patient outcomes and satisfaction, focusing on preventative medicine and holistic wellness.
- Maintain a manageable patient schedule, typically seeing approximately 18 patients per day, with administrative time built into your daily routine.
- Work exclusively during clinic hours, Monday through Friday, from 7:30 am to 5:00 pm, with no night or weekend shifts required.
- Engage with in-house consulting specialists, such as cardiology and podiatry, and refer patients as needed for comprehensive care.
- Participate in holistic health initiatives offered by the clinic, including acupuncture, Tai Chi, yoga, and nutrition services tailored to the needs of seniors.
- Contribute to the organization’s mission by supporting transportation services for patients in need, ensuring accessibility to clinic appointments throughout Richmond, VA.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered care models and a passion for serving the senior population.
- Strong interpersonal, communication, and organizational skills, with the ability to work collaboratively within a multidisciplinary team.
- Current, unrestricted medical license to practice as a physician in the state of Virginia or eligibility to obtain one.
