As a Primary Care Physician (MD/DO), you will manage a small patient panel, allowing for personalized attention and continuity of care. Our clinical model emphasizes value-based care, patient education, and preventative medicine, supported by an integrated care team including a dedicated Medical Assistant, Scheduler, and Referral Specialist. Primary Care Physician (MD/DO) – Summary
- Provide comprehensive outpatient primary care to a panel of up to 450 senior patients.
- Deliver value-based, preventative, and holistic medical care.
- Collaborate with a dedicated care team to ensure high-quality outcomes.

Duties & Responsibilities
- Conduct thorough patient assessments and manage chronic and acute health conditions in seniors, focusing on 3–5 chronic diseases per patient.
- Develop, implement, and monitor individualized care plans in alignment with value-based care principles.
- Provide preventive care, health education, and counseling to patients and families.
- Collaborate with in-house specialists (e.g., Cardiology, Podiatry), holistic health practitioners, and support staff to coordinate comprehensive care.
- Maintain detailed and accurate medical records according to organizational and regulatory standards.
- Participate in quality improvement initiatives and team meetings.
- Ensure timely completion of administrative tasks with dedicated schedule time.
- Refer patients to appropriate specialists when necessary and coordinate follow-up care.
- Adhere to ethical, legal, and professional standards in all clinical activities.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in FM, IM, or Geriatric Medicine.
- Commitment to delivering high-quality, compassionate, and patient-centered care.
- Strong communication, collaboration, and organizational skills.
