Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven healthcare organization in Bradenton, FL. Key Responsibilities
- Provide comprehensive, outpatient primary care to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Manage and coordinate care plans for patients, ensuring quality and continuity of care through evidence-based practices.
- Work collaboratively with an integrated team, including your own medical assistant, scheduler, and referral specialist, to deliver exceptional patient experiences.
- Maintain a patient schedule of approximately 18 patients per day, allowing for personalized care and meaningful patient relationships.
- Utilize value-based care principles to optimize patient health outcomes and participate in ongoing quality improvement initiatives.
- Complete administrative tasks within designated administrative time built into your schedule.
- Refer patients to in-house specialists as needed, including cardiology and podiatry, and coordinate access to holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure appropriate documentation and adherence to all clinical and regulatory standards.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered care for senior populations with complex medical needs.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
Work Schedule and Environment
- Full-time position based in Bradenton, FL.
- Clinic hours are Monday through Friday, 7:30 a.m. to 5:00 p.m.; last patient scheduled at 4:15 p.m.
- No nights or weekends required, supporting a healthy work-life balance.
- 100% outpatient care with administrative time included in daily schedule.
