As a Primary Care Physician (MD/DO), you will practice in a state-of-the-art clinical environment featuring consulting specialists onsite and holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling. The clinic is designed to foster collaboration between providers, care coordinators, and administrative staff, enabling seamless, patient-centered care. The role is 100% outpatient, with no nights or weekends required, ensuring that you maintain an excellent work-life balance while making a meaningful impact in the lives of the senior population.

Primary Care Physician (MD/DO) - Summary- Full-time, outpatient primary care role focused on seniors with chronic health needs
- Physician-led organization dedicated to value-based care and patient-centered outcomes

- Provide comprehensive primary care services to a panel of approximately 450 patients, primarily seniors managing multiple chronic conditions
- Conduct patient assessments, develop individualized care plans, and coordinate with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Collaborate with onsite consulting specialists in cardiology, podiatry, and other fields to ensure holistic patient care
- Utilize holistic health services available at the clinic, including acupuncture, Tai Chi, yoga, and nutrition counseling, to support patient wellness
- Maintain regular clinic hours Monday through Friday, with administrative time included in your schedule
- Adhere to value-based care principles and participate in quality improvement initiatives
- Ensure timely and accurate documentation of patient encounters and treatment plans
- Foster strong patient relationships that support health outcomes and patient satisfaction

- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
