Primary Care Physician (MD/DO) – St. Louis, MO – Full-time
We are seeking a dedicated and mission-driven Primary Care Physician (MD/DO) to join our physician-led, value-based primary care organization in St. Louis, MO. Our team is committed to delivering exceptional, affordable care to seniors, focusing on improving health outcomes and transforming the lives of the patients we serve. As a Primary Care Physician (MD/DO), you will play a vital role in delivering patient-centered care, fostering strong relationships, and leading clinical excellence in a collaborative environment.
Responsibilities
- Provide comprehensive outpatient primary care to a dedicated panel of senior patients, focusing on those with multiple chronic conditions.
- Manage a small patient panel (maximum 450 patients), enabling deeper relationships and personalized care.
- See approximately 18 patients per day during clinic hours, Monday through Friday, 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
- Collaborate with an integrated care team, including a dedicated medical assistant, scheduler, and referral specialist, to ensure coordinated and efficient patient care.
- Utilize value-based care principles to drive improved outcomes, quality, and patient satisfaction.
- Participate in regular administrative time for charting, care coordination, and professional development.
- Engage in partnership opportunities, with the majority of providers achieving partnership status within 12-18 months.
- Provide care in a 100% outpatient setting with no nights or weekends required, supporting an excellent work-life balance.
- Leverage in-house consulting specialists (Cardiology, Podiatry, etc.) and holistic health services (Acupuncture, Tai Chi, Yoga, Nutrition) to support comprehensive patient care.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong commitment to value-based care and a passion for serving seniors with multiple chronic diseases.
- Excellent interpersonal and communication skills, with the ability to work collaboratively in a team-based care model.
