Primary Care Physician (MD/DO) - Summary
- Full-time outpatient position focused on value-based primary care
- Serve a panel of senior patients with chronic health conditions

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients
- Conduct patient visits, averaging 18 patients per day, Monday through Friday
- Deliver care in a 100% outpatient setting with no nights or weekends required
- Utilize value-based care principles to improve patient outcomes and satisfaction
- Work collaboratively with a dedicated support team, including a medical assistant, scheduler, and referral specialist
- Participate in administrative activities during scheduled times
- Coordinate care with in-house consulting specialists in cardiology, podiatry, and more
- Incorporate holistic health services such as acupuncture, Tai Chi, yoga, and nutrition into patient care plans
- Ensure continuity of care and proactive management of chronic diseases (typically 3-5 per patient)
- Engage in continuous learning and professional development
Salary & Benefits
- Competitive base salary starting at $250,000 with additional performance-based bonuses
- Signing bonus up to $50,000
- Eligibility for partnership, typically achieved within 12-18 months (no buy-in required)
- Comprehensive benefits package including health, dental, vision, and supplemental benefits for provider and family

- Generous paid time off: 33 days PTO annually
- Continuing medical education allowance: $3,500 plus one week

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Commitment to value-based, patient-centered care
- Strong communication, interpersonal, and teamwork skills
- Desire to work with a senior patient population and manage chronic conditions
