Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care services to adult and senior patients, with a focus on those managing multiple chronic conditions
- Manage a daily schedule of approximately 18 patients in a 100% outpatient setting
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives aimed at improving patient outcomes
- Coordinate care with in-house specialists (cardiology, podiatry, etc.) and leverage holistic health services offered at the clinic
- Utilize administrative time built into the schedule to ensure thorough documentation and follow-up care
- Foster strong, supportive relationships with patients and their families to promote engagement in their health journey
- Commit to ongoing professional development and participation in team meetings and quality improvement activities

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
