Primary Care Physician (MD/DO) – Tampa, FL – Full-time Overview
Join a leading physician-led organization as a Primary Care Physician (MD/DO) in Tampa, FL. As part of a mission-driven team, you will help transform healthcare delivery for patients with complex chronic conditions, contributing to a collaborative environment focused on quality and patient-centered outcomes.
Responsibilities
- Provide high-quality, full-scope primary care services in an outpatient clinic setting to a panel of up to 450 patients, primarily seniors with multiple chronic diseases.
- Conduct thorough patient assessments, develop care plans, and manage ongoing treatment for adults with conditions such as hypertension, diabetes, and heart disease.
- Collaborate with an integrated care team, including a dedicated medical assistant, scheduler, and referral specialist, to ensure streamlined patient care.
- Utilize value-based care models to improve patient outcomes and maximize quality metrics.
- Maintain regular clinic hours from Monday through Friday, 7:30 a.m. to 5:00 p.m., with no scheduled nights or weekends.
- Balance clinical and administrative responsibilities with built-in administrative time during the workday.
- Engage with in-house consulting specialists, such as cardiology and podiatry, to coordinate holistic, multidisciplinary care.
- Promote holistic health services, including acupuncture, tai chi, yoga, and nutritional support, as part of patients’ wellness plans.
- Support the organization’s mission to deliver affordable, high-quality care to underserved seniors in Tampa, FL.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Dedication to providing patient-centered care and working with a multidisciplinary team.
- Experience or interest in caring for geriatric populations with multiple chronic conditions is preferred.
