Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led and mission-driven primary care organization in Tampa, FL. As a Primary Care Physician, you will be part of an innovative team committed to transforming healthcare delivery for the senior population. Our organization is focused on providing affordable, value-based care to seniors, ensuring better health outcomes and an improved patient experience. Key Responsibilities
- Provide comprehensive primary care services to a patient panel consisting primarily of seniors, most of whom manage 3-5 chronic diseases.
- Deliver patient-centered, value-based care in an outpatient clinic setting with an emphasis on preventative medicine, chronic disease management, and holistic wellness.
- Manage a maximum panel of 450 patients to ensure personalized and attentive care.
- Conduct patient visits (up to 18 per day), perform detailed assessments, diagnose medical conditions, and create tailored treatment plans.
- Collaborate with an in-house team of consulting specialists, including cardiology and podiatry, as well as holistic health experts offering acupuncture, tai chi, yoga, and nutrition services.
- Work closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to streamline patient care and optimize patient outcomes.
- Participate in value-based care initiatives designed to improve health outcomes and patient satisfaction.
- Maintain accurate and thorough documentation of patient encounters and medical histories.
- Follow a Monday through Friday schedule with clinic hours from 7:30 am to 5:00 pm, ensuring work/life balance with no night or weekend shifts.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine required.
- Board Certification or Board Eligibility in either FM, IM, or Geriatric Medicine.
- Commitment to delivering high-quality, value-based, and patient-focused care.
- Strong interpersonal and communication skills to effectively collaborate with patients, care teams, and consulting specialists.
- Ability to work in a fast-paced outpatient environment while managing a manageable patient panel size.
