You will work alongside a dedicated care team, including medical assistants, schedulers, and referral specialists, allowing you to focus on clinical excellence and meaningful patient relationships. Administrative time is built into your schedule, and you will benefit from a Monday through Friday workweek, with no nights or weekends required. The organization also provides door-to-door transportation for patients, ensuring reliable access to care.

Primary Care Physician (MD/DO) – Summary
- Provide comprehensive, value-based primary care to a panel of older adults in an outpatient clinic setting
- Work collaboratively with a supportive, interdisciplinary care team

Duties & Responsibilities
- Deliver patient-centered primary care to a small panel (maximum 450) of senior patients, most with multiple chronic conditions
- Conduct thorough patient assessments, develop care plans, and manage ongoing treatment in an outpatient setting
- Utilize value-based care principles to achieve optimal health outcomes
- Collaborate with in-house consulting specialists and holistic health providers
- Work closely with dedicated support staff, including your own medical assistant, scheduler, and referral specialist
- Document patient encounters accurately and timely within the electronic health record (EHR)
- Participate in quality improvement initiatives and care team meetings
- Ensure coordination of care, including referrals and follow-up, to optimize patient outcomes
- Contribute to maintaining a welcoming, respectful, and inclusive clinic environment
Salary & Benefits

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of a residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certification or eligibility in IM, FM, or Geriatric Medicine
- Commitment to value-based, patient-centered care
- Strong interpersonal, communication, and organizational skills
- Ability to work collaboratively within a multidisciplinary team
- Dedication to serving senior populations and managing complex, chronic conditions
