Primary Care Physician (MD/DO) – Summary
- Full-time, outpatient-only Primary Care Physician (MD/DO) role focused on geriatric and adult patient populations.

Duties & Responsibilities
- Provide comprehensive primary care to a senior population, focusing on prevention, chronic disease management, and holistic health.
- Manage a small patient panel (maximum 450 patients) to ensure quality and personalized care.
- Conduct thorough patient assessments, develop individualized care plans, and coordinate with in-house specialists in areas such as cardiology and podiatry.
- Utilize value-based care principles to drive improved patient outcomes.
- Participate in a Monday–Friday schedule with clinic hours from 7:30 AM to 5:00 PM; last patient scheduled at 4:15 PM.
- Enjoy dedicated administrative time during your workday to manage patient records and coordinate care.
- Collaborate with your own dedicated support team, including a medical assistant, scheduler, and referral specialist.
- Leverage in-house holistic health offerings, such as acupuncture, Tai Chi, yoga, and nutrition services, to support patient well-being.
- Ensure continuity of care through effective communication and coordination with all members of the healthcare team.
Salary & Benefits
