Primary Care Physician (MD/DO) - Summary- Full-time position focused on providing primary care to senior patients

- Deliver comprehensive outpatient primary care to a panel of up to 450 patients
- See approximately 18 patients per day in a Monday through Friday schedule
- Manage and coordinate care for seniors with multiple chronic conditions
- Utilize a value-based care model to improve patient outcomes
- Collaborate with a dedicated care team (medical assistant, scheduler, referral specialist)
- Integrate in-house specialist consultations and holistic health services into care plans
- Participate in partnership opportunities within the organization
- Maintain accurate and timely documentation and administrative duties
