Primary Care Physician (MD/DO) - Summary- Full-time position for a Primary Care Physician (MD/DO)

- Manage a small patient panel (up to 450 patients) to ensure personalized care
- Conduct outpatient visits for up to 18 patients per day
- Collaborate with a dedicated care team, including MA, scheduler, and referral specialist
- Develop and implement individualized care plans for seniors managing multiple chronic conditions
- Utilize value-based care principles to drive improved health outcomes
- Coordinate with in-house consulting specialists for comprehensive patient management

- Participate in administrative tasks with scheduled time set aside
- Maintain a regular Monday through Friday schedule; no nights or weekends
