Primary Care Physician (MD/DO) – Full-Time

As a Primary Care Physician (MD/DO) – Full-Time, you will work collaboratively with a dedicated care team, including medical assistants, schedulers, and referral specialists, all supported by a proactive leadership group. Your panel size will be thoughtfully managed to ensure you can provide focused, personalized attention to each patient, with an average of 10-12 patients seen per day and a maximum panel of 650. This allows you to build trust and continuity, leading to better outcomes and higher patient satisfaction.
The position offers a structured weekday schedule with minimal call requirements, contributing to an excellent work-life balance. The call schedule is shared equitably among providers, ensuring reasonable expectations and ample support. - Participate in value-based initiatives, focusing on HEDIS compliance and reducing hospital admissions
- Maintain accurate, timely, and compliant medical records
- Engage in patient education and health promotion activities
- Participate in a shared call schedule with other providers

- MD or DO degree from an accredited institution
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and DEA registration
- Proficiency in common office procedures: suturing, joint injections, skin biopsies, wound care
- Fellowship in Geriatrics is a plus but not required
- Strong commitment to patient-centered care and team collaboration
- Experienced or newly trained physician passionate about senior care
- Comfortable managing patients with multiple, complex chronic illnesses
- Team-oriented, communicative, and proactive in patient advocacy
- Desire to work in a value-based care environment
- The center provides free wellness activities and transportation support for patients
- New clinic opening in May 2025
