Primary Care Physician (MD/DO) - Summary
- Deliver high-quality, patient-centered primary care in an outpatient setting.
- Work Monday through Friday, with regular daytime hours to support work-life balance.
- Care for a dedicated panel of senior patients, typically managing 3-5 chronic health conditions per individual.
- Engage in a value-based care delivery model focused on outcomes and patient satisfaction.
Duties & Responsibilities
- Conduct comprehensive primary care visits for a daily average of 10-12 patients, focusing on seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Maintain a manageable panel size, with a maximum of 650 patients, to ensure quality care and attention to each patient.
- Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists for optimal patient support.
- Participate in a rotating call schedule determined by the provider team size.
- Utilize evidence-based practices and adhere to HEDIS compliance standards to achieve quality outcomes.
- Contribute to a positive clinic culture by embodying organizational values and fostering a supportive environment for both patients and staff.
- Support patient wellness initiatives, including community health programs, exercise, and art activities designed for seniors.
- Engage in ongoing professional development and maintain current clinical skills relevant to office-based procedures such as suturing, joint injections, skin biopsies, and wound care.

- Competitive base salary with total earning potential exceeding $300,000, including performance bonuses.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license and current DEA registration.
- Proficiency in common office procedures including suturing, joint injections, skin biopsies, and wound care management.
- Fellowship training in Geriatrics is a plus but not required.
