Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 650 senior patients, many with multiple chronic conditions such as heart disease, diabetes, and hypertension
- Deliver care in an outpatient setting, averaging 10-12 patient encounters per day
- Lead and collaborate with your care team, which includes a medical assistant, scheduler, referral specialist, and full leadership support
- Participate in a value-based care model focused on improving patient satisfaction, HEDIS compliance, and reducing hospital admissions
- Utilize proficient PC skills for office procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Maintain medical records and ensure accurate documentation of all patient encounters
- Participate in call schedules as determined by the provider team (e.g., 1:32 weeks, based on physician count)
- Promote wellness through patient education and engagement in health-promoting activities offered at the clinic

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures relevant to primary care
- Fellowship training in Geriatrics is considered a plus, but not required
