In this full-time Primary Care Physician (MD/DO) role, you will be part of a collaborative care team with dedicated support staff, including medical assistants, schedulers, and referral specialists. Our clinics are equipped to ensure that every patient receives attentive care and that our providers can focus on meaningful, long-term patient relationships. Primary Care Physician (MD/DO) – Summary
- Provide outpatient primary care services focused on a senior patient population with complex health needs.
- Work within a value-based care model to optimize patient outcomes and satisfaction.
- Collaborate with a dedicated care team to deliver coordinated, high-quality care.
- Support the organization’s mission to foster wellness, respect, and community engagement among patients and staff.
Duties & Responsibilities
- Serve as the primary point of care for a panel of up to 650 senior patients with chronic and complex conditions.
- Conduct thorough patient assessments, develop and implement care plans, and monitor progress toward health goals.
- Practice standard outpatient clinic hours, typically Monday through Friday, 8:00 AM to 5:00 PM.
- Utilize electronic medical records and maintain accurate, up-to-date patient documentation.
- Participate in a fair, rotational call schedule determined by the provider group size.
- Engage patients in preventive care, wellness activities, and disease management programs offered by the clinic.
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Collaborate closely with MAs, referral specialists, and other team members to ensure seamless patient experiences.
- Contribute to quality initiatives and meet value-based metrics, including patient satisfaction, compliance, and reduced hospitalizations.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license in good standing.
- Current DEA registration.
- Proficiency in outpatient office procedures, including suturing, joint injections, skin biopsies, and wound care.
- Fellowship training in Geriatrics is a plus, but not required.
