Primary Care Physician (MD/DO) – Summary
- Deliver evidence-based, patient-centered primary care in a modern outpatient clinic setting.
- Focus on care for seniors with multiple chronic conditions.
- Participate in a value-based care model emphasizing quality, satisfaction, and health outcomes.
- Work collaboratively with a dedicated care team and supportive leadership.
Duties & Responsibilities
- Provide comprehensive primary and preventive care to a diverse adult and senior patient population.
- Manage a patient panel of up to 650 individuals, averaging 10-12 patient visits per day.
- Diagnose, treat, and manage chronic health conditions, including heart disease, diabetes, and hypertension.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Coordinate care with multidisciplinary team members, including medical assistants and referral specialists.
- Utilize electronic health records and clinic technology to document patient care and communicate with the care team.
- Participate in quality improvement initiatives and meet performance goals related to patient satisfaction, HEDIS compliance, and reduced hospitalizations.
- Engage in community health and wellness programs offered by the clinic, such as exercise and art activities for patients.
- Adhere to clinic policies, professional standards, and applicable regulations at all times.
- Participate in a rotating call schedule as determined by the provider team.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Current, unrestricted, and active medical license.
- Valid DEA registration required.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and wound care.
- Fellowship training in Geriatrics is a plus, but not required.
- Commitment to providing patient-centered, compassionate care aligned with value-based healthcare principles.
