Primary Care Physician (MD/DO) - Summary
- Provide comprehensive, patient-focused primary care services using a value-based care approach.
- Serve a panel of adult and senior patients with complex chronic conditions (e.g., heart disease, diabetes, hypertension).
- Collaborate with dedicated care teams, including medical assistants, schedulers, and referral specialists.
- Leverage innovative care delivery models to improve patient satisfaction and health outcomes.
Duties & Responsibilities
- Conduct thorough patient assessments and develop individualized care plans for a diverse, predominantly senior population.
- Manage a patient panel (up to 650 individuals) with an average of 10-12 patients seen per day, ensuring continuity and quality of care.
- Perform outpatient clinical services, including office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Lead and collaborate with a multidisciplinary team to coordinate patient care and optimize health outcomes.
- Participate in call schedules based on provider availability (e.g., 1:32 weeks), ensuring comprehensive patient coverage.
- Utilize electronic health records and maintain accurate, up-to-date documentation for all patient interactions.
- Promote best practices in disease prevention, health education, and chronic disease management.
- Engage in ongoing professional development and adhere to all federal, state, and organizational compliance standards.

Qualifications & Requirements
- Medical degree (MD or DO) with current, active, and unrestricted medical license.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- DEA registration and authority to prescribe medication in accordance with state and federal regulations.
- Proficiency in outpatient clinical procedures (suturing, joint injections, biopsies, wound care).
- Strong commitment to patient-centered, value-based care principles.
- Fellowship in Geriatrics is a plus, but not required.
- Excellent communication, collaboration, and leadership skills.
