Primary Care Physician (MD/DO) – Cleveland, OH – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Cleveland, OH. As a Primary Care Physician, you will play a pivotal role in delivering high-quality, patient-centered care in an outpatient clinic environment. Responsibilities
- Provide comprehensive primary care to a panel of senior patients, many of whom present with multiple complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care in a value-based model, emphasizing patient satisfaction, adherence to clinical guidelines, and proactive management of patient health to reduce hospitalizations and improve outcomes.
- Manage an outpatient patient load averaging 10–12 patients per day, with a maximum panel size of 650, ensuring personalized attention and continuity of care.
- Collaborate with a dedicated care team that includes a medical assistant, scheduler, referral specialist, and full support from clinic leadership to optimize patient care and administrative efficiency.
- Utilize your clinical skills in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Participate in a shared call schedule, which is distributed among all physicians in the market, promoting a healthy work-life balance.
- Uphold the company’s core values and contribute to a positive, patient-focused clinic culture.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) is required.
- Fellowship training in Geriatrics is considered a plus, but is not mandatory.
- Current, unrestricted, and active license to practice medicine in Ohio.
- Current DEA registration.
- Proficiency in key office procedures including suturing, joint injections, skin biopsies, and basic wound care.
