Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Deliver comprehensive outpatient primary care to senior patients, managing chronic conditions such as heart disease, diabetes, and hypertension
- Provide patient-centered care, focusing on wellness, prevention, and continuity
- Maintain a manageable daily patient load (10-12 patients/day) and panel size (up to 650 patients)
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Participate in a value-based care model, emphasizing patient satisfaction, quality metrics (HEDIS compliance), and reducing hospitalizations
- Perform standard office procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Maintain accurate and timely documentation in the electronic medical record
- Participate in call coverage, with frequency determined by the number of providers in the market
- Promote wellness through participation in and support of patient activities offered at the clinic

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and current DEA registration
- Fellowship training in Geriatrics is a plus, but not required
- Proficient in common office procedures: suturing, joint injections, skin biopsies, and basic wound care
- Strong commitment to patient-centered, evidence-based care
- Excellent teamwork, communication, and organizational skills
