As a Primary Care Physician (MD/DO) in our Montgomery, AL clinic, you will lead a dedicated care team, including a medical assistant, scheduler, and referral specialist, all working collaboratively to support patient needs. You will manage a focused panel of seniors, allowing you to develop strong, ongoing relationships with your patients and deliver comprehensive, individualized care. Primary Care Physician (MD/DO) - Summary- Full-time, outpatient primary care role
- Value-based care model with a focus on senior patient population

- Provide comprehensive primary care to adult and senior patients with complex health needs
- Manage a patient panel of up to 650 individuals
- Average of 10-12 patients per day, allowing for quality, personalized care
- Lead and collaborate with a dedicated care team (MA, scheduler, referral specialist)
- Utilize value-based care principles to achieve optimal patient outcomes
- Participate in quality improvement initiatives, including HEDIS compliance and reducing hospitalizations
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and wound care
- Maintain accurate and timely medical records
- Participate in a rotational call schedule based on the number of providers in the market
- Competitive base salary starting at $260,000, with total earning potential exceeding $312,000
- Performance bonuses based on patient satisfaction, adherence to company values, HEDIS metrics, and reduced hospitalizations

- Current, unrestricted medical license and DEA registration
- Proficiency in office-based procedures: suturing, joint injections, skin biopsies, wound care
- Fellowship in Geriatrics is a plus but not required
