Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive outpatient primary care to a panel of adult and senior patients, focusing on those with complex chronic conditions.

Duties & Responsibilities
- Provide high-quality, evidence-based primary care to a panel of up to 650 patients, with a daily average of 10–12 patient visits.
- Focus on managing complex chronic conditions such as heart disease, diabetes, hypertension, and more, particularly within the senior population.
- Coordinate care with a dedicated team, including medical assistants, schedulers, and referral specialists, ensuring seamless patient experiences.
- Participate in a value-based care structure with incentives tied to patient satisfaction, adherence to clinical guidelines (HEDIS compliance), and reduction of preventable hospitalizations.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as clinically appropriate.
- Engage in community health initiatives including wellness activities, exercise programs, and art therapy for patients, supporting holistic care.
- Maintain accurate documentation and compliance with all regulatory and organizational standards.
- Participate in a manageable call schedule, coordinated with peers to ensure work-life balance.

- Competitive base salary with total earning potential exceeding $320,000, including performance-based bonuses.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM); additional Geriatrics fellowship preferred but not required.
- Active, unrestricted medical license and current DEA registration.
- Competency in common office-based procedures (suturing, joint injections, skin biopsies, basic wound care).
- Strong commitment to value-based care and patient-centered outcomes.
- Excellent communication, teamwork, and organizational skills.
- Proficiency in electronic medical records and basic computer skills.
