Primary Care Physician (MD/DO) - Summary
- Provide comprehensive, patient-centered primary care in a modern outpatient clinic.
- Serve a senior patient population, many of whom manage multiple chronic conditions.
- Collaborate with a dedicated care team to deliver high-quality care and achieve value-based outcomes.

Duties & Responsibilities
- Deliver evidence-based primary care for adult and senior patients with complex health needs.
- Maintain a daily patient load of 10-12 individuals, ensuring sufficient time for each consultation.
- Oversee a patient panel with a maximum size of 650 patients.
- Coordinate care with multidisciplinary teams, including medical assistants, schedulers, and referral specialists.
- Utilize best practices in clinical procedures such as suturing, joint injections, skin biopsies, and wound care.
- Promote preventive health measures and chronic disease management.
- Ensure compliance with company values, HEDIS metrics, and patient satisfaction goals.
- Participate in a rotating call schedule with support from the broader provider network.
- Engage in value-based care initiatives and continuous quality improvement activities.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license and current DEA registration.
- Proficient in common office-based procedures (suturing, joint injections, skin biopsies, wound care).
- Experience in geriatrics or managing patients with multiple chronic diseases preferred.
