Primary Care Physician (MD/DO) – Summary- Full-time outpatient position focused on value-based, senior primary care
- Work with a collaborative care team in a clinic designed for patient comfort and engagement
- Manageable patient panel and daily volume to foster quality interactions

- Provide comprehensive primary care services to a senior patient population, focusing on chronic disease management and preventative care

- Lead a care team consisting of a medical assistant, scheduler, and referral specialist
- Participate in a call schedule determined by the number of providers in the market
- Adhere to clinical best practices and organizational standards, including HEDIS compliance
- Engage in ongoing communication with patients, families, and multidisciplinary team members
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Promote wellness and preventive services, including participation in onsite activities for seniors

- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Current, unrestricted, active medical license
- Current DEA registration
- Proficient in outpatient procedures (suturing, joint injections, skin biopsies, wound care)
- Fellowship in Geriatrics is a plus but not required
