Primary Care Physician (MD/DO) – Full-Time Overview
Join a forward-thinking healthcare organization as a Primary Care Physician (MD/DO) and play a vital role in delivering comprehensive outpatient care to a diverse senior patient population. This full-time opportunity is based in Sarasota, FL, a city known for its beautiful Gulf Coast setting and vibrant community. As a Primary Care Physician, you will be part of a mission-driven team focused on promoting wellness, providing patient-centered care, and improving health outcomes through a value-based care model.
Responsibilities
- Provide high-quality primary care services to an adult and senior patient population, managing complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care in an outpatient clinic setting, fostering strong patient-provider relationships and promoting continuity of care.
- Work collaboratively within a multidisciplinary care team, including medical assistants, schedulers, referral specialists, and support from clinic leadership.
- Maintain a manageable patient panel of up to 650, with an average of 10-12 patients per day, ensuring adequate time for each patient encounter.
- Utilize evidence-based practices and adhere to clinical guidelines to achieve optimal patient outcomes, including HEDIS compliance.
- Actively participate in quality improvement initiatives, focusing on patient satisfaction, reduction of hospitalizations, and adherence to organizational values.
- Engage patients in preventive care, wellness education, and chronic disease management, leveraging in-clinic programs such as exercise, art, and health activities.
- Participate in a call schedule determined by the number of physicians in the market (for example, 1:32 weeks if there are 32 providers).
- Document all patient care activities accurately and timely using electronic health records.
- Adhere to all relevant regulatory, legal, and ethical standards in the practice of medicine.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine.
- Valid DEA registration.
- Fellowship training in Geriatrics is a plus but not required.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Strong commitment to patient-centered care and collaborative teamwork.
- Excellent communication and organizational skills.
