Primary Care Physician (MD/DO) - Summary
- Provide comprehensive outpatient primary care to a senior patient population with complex health conditions.
- Practice in a value-based care environment focused on quality outcomes and patient satisfaction.

Duties & Responsibilities
- Deliver high-quality, compassionate medical care to patients, primarily seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Conduct thorough physical examinations, diagnose illnesses, and develop individualized care plans.
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care as appropriate.
- Work closely with a designated care support team including medical assistants, schedulers, and referral specialists.
- Emphasize preventive care, wellness education, and chronic disease management.
- Utilize electronic medical records to accurately document patient encounters and coordinate care.
- Participate in care team meetings and contribute to continuous quality improvement initiatives.
- Engage in regular communication with patients and their families to foster trust and continuity of care.
- Maintain compliance with clinical protocols, regulatory standards, and organizational values.
- Participate in an equitable call schedule, supported by a large physician team.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted medical license to practice medicine.
- Active and valid DEA registration.
- Proficient in outpatient office procedures relevant to primary care.
- Fellowship training in Geriatrics is a plus but not required.
