Key Responsibilities- Deliver comprehensive primary care services to a predominantly senior patient population with multiple chronic conditions, including heart disease, diabetes, and hypertension.
- Practice exclusively in an outpatient setting, ensuring high-quality care and continuity for all patients.
- Manage an average daily patient volume of 10–12, with a maximum panel size of 650, allowing for personalized and attentive care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, referral specialist, and full leadership support, to coordinate patient care and streamline clinic operations.
- Participate in a value-based care model, focusing on patient satisfaction, adherence to clinical guidelines (such as HEDIS compliance), and reducing unnecessary hospitalizations.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Engage in activities that promote patient wellness, such as supporting health-related programs and encouraging participation in community-based initiatives offered at the clinic.
- Contribute to a positive patient experience by embodying the organization’s core values and maintaining high patient satisfaction ratings.
- Participate in the center’s call schedule, which is equitably distributed among providers—e.g., approximately 1 week out of every 32 if there are 32 physicians in the market.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active medical license to practice medicine.
- Current DEA registration.
- Proficiency in office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is highly valued but not required.
