Primary Care Physician (MD/DO) - Summary
- Full-time role providing comprehensive primary care for a diverse senior patient panel, focusing on chronic disease management and wellness.
- Work within a value-based care model to drive patient satisfaction and improved health outcomes.
- Collaborate with a dedicated care team including a medical assistant, scheduler, referral specialist, and supportive leadership.
- Deliver care in a modern, community-focused clinic environment with strong organizational support.
Duties & Responsibilities
- Provide direct patient care for a panel of seniors with complex health needs, including those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Maintain an average daily census of 10-12 patients to ensure personalized and high-quality care.
- Engage in outpatient primary care activities Monday through Friday, 8:00 am to 5:00 pm.
- Participate in a fair and manageable call rotation, structured according to the number of providers in the market.
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care as appropriate.
- Utilize electronic health records and PC skills to document visits, coordinate care, and ensure compliance with HEDIS measures.
- Collaborate closely with your care team to manage a maximum panel size of 650 patients, ensuring robust continuity of care and patient engagement.
- Contribute to company initiatives that promote patient satisfaction, compliance, and the reduction of unnecessary hospitalizations.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) is mandatory.
- Active, unrestricted medical license and current DEA registration.
- Experience or proficiency in common office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
- Strong interpersonal, communication, and organizational skills with a commitment to delivering patient-centered care.
- Comfort with electronic medical records and digital health technologies.
