Primary Care Physician (MD/DO) – Summary
- Deliver outpatient primary care services to a senior patient population with multiple chronic health conditions
- Work collaboratively within a multidisciplinary care team
- Participate in a value-based care model focused on patient outcomes, satisfaction, and quality metrics
- Enjoy a predictable schedule with no weekend requirements
Duties & Responsibilities
- Provide comprehensive medical care for adult and senior patients, managing chronic and acute medical conditions such as heart disease, diabetes, and hypertension
- Conduct routine physical exams, preventive care, and health screenings
- Perform office-based procedures including suturing, joint injections, skin biopsies, and basic wound care
- Coordinate care with other healthcare professionals, including specialists and ancillary staff
- Maintain accurate and timely medical records in compliance with organizational standards
- Engage patients in their health and wellness, promoting participation in onsite programs such as exercise and art activities
- Work with a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive leadership support
- Participate in call rotations as assigned, based on the number of providers in the market
- Contribute to the achievement of organizational goals for patient satisfaction, HEDIS compliance, and reduction of hospital sick days

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) – required
- Fellowship training in Geriatrics is a plus but not required
- Active, unrestricted license to practice medicine
- Current DEA registration
- Proficiency in office procedures including suturing, joint injections, skin biopsies, and basic wound care
