As a Primary Care Physician (MD/DO), you will play a pivotal role in a value-based care model, guiding patients with complex chronic conditions through their healthcare journey. Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 650 patients, averaging 10–12 patient appointments per day in a clinic setting
- Focus on managing seniors with complex health needs including heart disease, diabetes, and hypertension
- Deliver care aligned with value-based performance metrics, including patient satisfaction, adherence to quality standards, and reduction of hospitalizations
- Collaborate closely with a care team including medical assistants, schedulers, and referral specialists
- Participate in a supportive call schedule distributed across the provider team
- Utilize proficient office-based procedure skills such as suturing, joint injections, skin biopsies, and basic wound care
- Engage in patient and family education to support wellness and preventive care
- Uphold clinical documentation and regulatory compliance standards
- Contribute to the clinic’s culture by embodying organizational values and fostering a welcoming, respectful environment for all patients and staff

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board certification or eligibility in Family Medicine (FM) or Internal Medicine (IM) is required
- Fellowship in Geriatrics is a plus but not mandatory
- Active, unrestricted medical license and current DEA registration
- Proficiency in common office procedures, such as suturing, joint injections, skin biopsies, and basic wound care
