As a Primary Care Physician (MD/DO), you will manage a small patient panel, allowing for more personalized care and deeper connections with your patients. The clinic operates with a strong emphasis on work-life balance, offering a Monday through Friday schedule with no nights or weekends required. Primary Care Physician (MD/DO) – Summary
- Full-time position focusing on comprehensive, value-based care for senior patients

Duties & Responsibilities
- Manage a patient panel of up to 450 individuals, primarily seniors with multiple chronic conditions
- Provide high-quality, comprehensive primary care in an outpatient clinic setting
- Coordinate care with a dedicated team including medical assistants, schedulers, and referral specialists
- Utilize value-based care models to deliver effective and efficient healthcare
- Conduct patient visits, averaging 18 per day, with time allotted for administrative tasks
- Collaborate with in-house consulting specialists such as cardiology and podiatry, as well as holistic health providers (e.g., acupuncture, yoga, nutrition)
- Participate in quality improvement initiatives and population health management efforts
- Ensure continuity of care and strong patient-provider relationships

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine

- Strong interpersonal and communication skills
