Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven healthcare organization in Hampton, VA. This full-time, outpatient role is designed for providers who are passionate about delivering value-based care and making a meaningful difference in the lives of senior patients. As a Primary Care Physician (MD/DO), you will be at the forefront of transforming healthcare for seniors with complex health needs, while working in a supportive and collaborative environment.
Key Responsibilities
- Provide comprehensive primary care to a dedicated panel of up to 450 senior patients, focusing on those with multiple chronic conditions.
- Deliver patient-centered, value-based care emphasizing prevention, chronic disease management, and holistic wellness.
- Utilize a multidisciplinary team including your own Medical Assistant, Scheduler, and Referral Specialist to optimize patient care and administrative efficiency.
- Conduct patient visits in a 100% outpatient setting, managing an average of 18 patients per day.
- Work Monday through Friday during clinic hours (7:30am – 5:00pm), with no nights or weekends required, supporting work/life balance.
- Incorporate administrative time into your schedule to ensure thorough documentation and care coordination.
- Collaborate with in-house consulting specialists such as cardiology and podiatry, and refer patients to holistic health services offered onsite, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Serve a patient population comprised primarily of seniors managing three to five chronic diseases, with door-to-door transportation available for those in need.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Commitment to providing high-quality, value-based care to a diverse senior population.
- Open to candidates requiring employment visa sponsorship.
- Strong interpersonal skills and a collaborative approach to patient care.
