Working in Kenner, LA provides the opportunity to serve a diverse community with access to a range of supportive health resources.
Key Responsibilities
- Provide comprehensive primary care services to an assigned panel of patients, focusing primarily on seniors with multiple chronic conditions.
- Deliver patient-centered, value-based care in a 100% outpatient clinic setting, ensuring continuity and quality of care.
- Manage a small patient panel with a maximum of 450 patients, allowing for individualized attention and relationship-building.
- Conduct patient visits, averaging approximately 18 patients per day, with a strong emphasis on preventive medicine and chronic disease management.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes and streamline care coordination.
- Utilize administrative time included in your schedule to complete documentation and coordinate care.
- Participate in a Monday through Friday work schedule, with clinic hours from 7:30 AM to 5:00 PM, ensuring a healthy work/life balance with no required nights or weekends.
- Engage in value-based care initiatives, supporting organizational goals to improve quality metrics and patient satisfaction.
- Access in-house consulting specialists, such as cardiology and podiatry, to provide comprehensive and collaborative care for complex cases.
- Offer holistic health services, including acupuncture, tai chi, yoga, and nutrition, to support patients’ overall well-being.
- Ensure accessibility for all patients, with door-to-door transportation services available for those in need.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered care to seniors with complex chronic conditions.
- Strong communication, collaboration, and organizational skills.
- Ability to work effectively in a multidisciplinary team environment.
- Open to candidates requiring visa sponsorship.
