Primary Care Physician (MD/DO) Overview
We are seeking a compassionate and dedicated Primary Care Physician (MD/DO) to join our physician-led, mission-driven organization in Columbus, OH. Responsibilities
- Provide comprehensive primary care services to a dedicated panel of senior patients, focusing on preventive care, disease management, and wellness initiatives.
- Manage a small patient panel (maximum 450 patients) to ensure personalized, attentive care and meaningful patient relationships.
- Conduct patient visits in an outpatient clinic setting, with a typical daily schedule of 18 patients, Monday through Friday, from 7:30 a.m. to 5:00 p.m. (last appointment scheduled at 4:15 p.m.).
- Collaborate with an assigned care team, including a Medical Assistant, Scheduler, and Referral Specialist, to support seamless patient care and administrative efficiency.
- Engage in value-based care initiatives to improve patient outcomes, particularly for seniors managing multiple chronic conditions.
- Refer patients to in-house consulting specialists (such as Cardiology and Podiatry) as appropriate, and coordinate with holistic health services including acupuncture, tai chi, yoga, and nutrition counseling.
- Participate in regular administrative time built into the schedule to ensure proper documentation and follow-up.
- Utilize patient transportation services offered by the clinic to promote access to care.
- Maintain compliance with clinical protocols, organizational policies, and regulatory standards.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligible for medical licensure in the state of Ohio.
- Experience or interest in managing complex senior populations with multiple chronic conditions is preferred.
- Committed to practicing evidence-based, value-driven medicine in a collaborative, team-oriented environment.
- Open to candidates requiring visa sponsorship.
