Primary Care Physician (MD/DO) - Summary
- Directly responsible for providing primary care services in an outpatient clinic setting, focusing on a senior patient population.
- Work collaboratively within a multidisciplinary team to deliver value-based healthcare.
- Maintain a manageable patient panel, with a focus on quality of care and patient satisfaction.
- Support organizational mission to improve healthcare access and outcomes for seniors.
Duties & Responsibilities
- Deliver comprehensive primary care to a maximum of 18 patients per day, ensuring continuity and quality of care.
- Manage a small patient panel (up to 450 patients), allowing for personalized and attentive care.
- Lead and coordinate care with a dedicated clinical support team, including a Medical Assistant, Scheduler, and Referral Specialist.
- Participate in value-based care initiatives and quality improvement programs.
- Utilize in-house consulting specialists (e.g., Cardiology, Podiatry) and holistic health services (Acupuncture, Tai Chi, Yoga, Nutrition) to enhance patient outcomes.
- Maintain accurate and timely documentation of patient encounters and care plans.
- Engage in interdisciplinary case discussions and contribute to a culture of teamwork and collaboration.
- Participate in administrative activities, with dedicated time included in the schedule.
- Provide care exclusively in an outpatient setting—no nights, weekends, or inpatient responsibilities required.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Eligible for medical licensure and credentialing.
