Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) for a full-time opportunity in Norfolk, VA. Responsibilities
- Provide comprehensive outpatient primary care services to a panel of senior patients, focusing on the management of chronic diseases and preventive care.
- Maintain a patient panel with a maximum of 450 individuals to ensure high-quality, personalized care.
- Conduct thorough patient assessments, develop and implement treatment plans, and monitor ongoing care for seniors typically managing 3-5 chronic conditions.
- Collaborate with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist to facilitate coordinated care and streamlined patient experiences.
- Participate in value-based care initiatives, emphasizing improved health outcomes and patient satisfaction.
- Utilize in-house consulting specialists, such as Cardiology and Podiatry, as needed for patient care coordination.
- Leverage holistic health services, including acupuncture, tai chi, yoga, and nutrition counseling, to support patient well-being.
- Ensure timely and accurate documentation within the electronic health record system.
- Allocate administrative time within the work schedule to support efficient clinical operations.
- Adhere to a Monday – Friday clinic schedule, with no required nights or weekends, supporting work-life balance.
Qualifications
- Medical Degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Current and unrestricted medical license to practice as a Primary Care Physician in the state of Virginia, or eligibility to obtain.
- Commitment to value-based care and improving health outcomes for senior populations.
- Strong communication, organizational, and teamwork skills.
- Open to visa candidates.
