Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Deliver high-quality, patient-centered primary care for a panel of up to 450 patients, consisting primarily of seniors with multiple chronic diseases
- Conduct patient visits, averaging 18 patients per day, during regular clinic hours Monday through Friday
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient care and outcomes
- Utilize value-based care models and evidence-based guidelines to manage chronic conditions and promote preventive health
- Participate in regular team meetings, quality improvement initiatives, and ongoing professional development activities
- Engage with in-house consulting specialists and holistic health services to provide comprehensive, interdisciplinary care
- Ensure all documentation is accurate, timely, and compliant with company standards and regulatory requirements

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
