Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient-based primary care practice
- Serve a senior patient population with multiple chronic conditions
- Work within a value-based care model focused on patient outcomes

Duties & Responsibilities
- Provide comprehensive primary care to a dedicated patient panel (maximum 450 patients)
- Conduct routine and preventive care visits, manage chronic conditions, and coordinate specialty care as needed
- See an average of 18 patients per day, allowing for in-depth visits and strong patient relationships
- Utilize value-based care principles to improve patient outcomes and reduce unnecessary utilization
- Work closely with a dedicated care team, including your own medical assistant, scheduler, and referral specialist
- Participate in regular care team meetings and quality improvement initiatives
- Document patient encounters accurately and efficiently using electronic health records
- Refer patients to in-house specialists (e.g., cardiology, podiatry) and holistic health services as appropriate
- Engage in ongoing professional development and education

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
