As a Primary Care Physician (MD/DO), you will manage a small, manageable patient panel, focusing on seniors with multiple chronic conditions. The role emphasizes value-based care, ensuring every patient receives attention and comprehensive treatment. You will work with a supportive team that includes your own medical assistant, scheduler, and referral specialist, allowing you to focus on patient care and outcomes. This is a 100% outpatient position with no night or weekend shifts, giving you the opportunity to maintain an excellent work/life balance. Primary Care Physician (MD/DO) - Summary- Full-time outpatient primary care role managing a panel of senior patients with chronic conditions
- Collaborative, mission-driven, physician-led organization focused on transforming senior care

- Deliver comprehensive primary care to a panel of up to 450 senior patients
- Conduct patient assessments, diagnose, and manage multiple chronic diseases
- Work closely with an assigned team including a medical assistant, scheduler, and referral specialist
- Utilize value-based care models to achieve positive health outcomes and patient satisfaction
- See an average of 18 patients per day in a fully outpatient setting
- Participate in administrative duties with scheduled time for documentation and care coordination
- Collaborate with in-house consulting specialists, such as cardiology and podiatry
- Engage in holistic health initiatives like acupuncture, Tai Chi, yoga, and nutrition counseling offered by the clinic
- Maintain compliance with all applicable laws, regulations, and policies

- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
