Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time position in North Miami, FL. As part of a physician-led, mission-driven healthcare organization, you will play a vital role in delivering high-quality, value-based care to senior patients. This position offers the chance to work with a supportive team in a modern, patient-centered outpatient clinic focused on improving health outcomes for the local community.
Responsibilities
- Provide comprehensive primary care to a small panel of up to 450 senior patients, focusing on those with multiple chronic conditions.
- Conduct thorough patient assessments, develop individualized care plans, and manage ongoing treatment for adults and seniors.
- See an average of 18 patients per day during clinic hours, which run Monday through Friday from 7:30 a.m. to 5:00 p.m., with the last patient scheduled at 4:15 p.m.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure seamless patient care and efficient clinic operations.
- Utilize a value-based care approach to improve health outcomes, emphasizing preventive measures and holistic health services.
- Participate in administrative tasks and team meetings as allocated within your schedule, supporting a strong work-life balance.
- Refer patients to in-house consulting specialists, such as cardiology and podiatry, as needed, and coordinate holistic services including acupuncture, tai chi, yoga, and nutrition counseling.
- Provide outpatient care only—no nights or weekends are required.
- Ensure patient transportation needs are addressed through complimentary transportation services for those who require it.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, patient-centered care to a diverse senior population, including those with multiple chronic diseases.
- Strong interpersonal, communication, and organizational skills.
- Willingness to work collaboratively within a multidisciplinary team environment.
