Primary Care Physician (MD/DO) - Summary
- Full-time primary care provider position focused on the senior population
- Work collaboratively within a physician-led, mission-driven organization
- Provide comprehensive, value-based care to patients with complex chronic conditions
- Contribute to an environment dedicated to improving the lives of seniors
Duties & Responsibilities
- Manage a panel of up to 450 senior patients, focusing on high-quality, patient-centered care
- Conduct thorough patient assessments and develop individualized care plans
- Deliver care to approximately 18 patients per day in an outpatient clinic setting
- Utilize a dedicated support team including a medical assistant, scheduler, and referral specialist
- Participate in value-based care initiatives, focusing on outcomes for seniors with multiple chronic diseases
- Work Monday through Friday, with clinic hours from 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm)
- Ensure administrative responsibilities are completed within scheduled time
- Collaborate with in-house specialists across cardiology, podiatry, and more, as well as holistic health services
- Promote preventive health, wellness, and patient education initiatives
- Maintain accurate documentation and uphold standards of clinical excellence
Salary & Benefits
- Competitive base salary of $250,000+, plus performance-based incentive bonuses
- Signing bonus available (up to $50,000)

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to practicing value-based, patient-centered care
- Strong communication and teamwork skills, with a collaborative approach to care delivery
