This role offers an opportunity to work in a collaborative team environment, supported by administrative and clinical staff, including a dedicated Medical Assistant, Scheduler, and Referral Specialist. The Primary Care Physician (MD/DO) will serve a small, manageable patient panel, ensuring quality time and personalized attention for each patient. The position is 100% outpatient, with no nights or weekends required, supporting an excellent work/life balance. Our clinics are equipped with in-house consulting specialists and holistic health services, including cardiology, podiatry, acupuncture, tai chi, yoga, and nutrition programs. Patients benefit from door-to-door transportation services at no additional cost, furthering our commitment to accessibility and comprehensive care.

Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 senior patients, focusing on individuals with multiple chronic conditions
- Manage a schedule of approximately 18 patients per day, ensuring personalized, high-quality care
- Conduct patient assessments, develop and implement treatment plans, and manage ongoing care coordination
- Collaborate with in-house consulting specialists (e.g., Cardiology, Podiatry), holistic health practitioners, and support staff
- Utilize value-based care principles to improve patient outcomes and satisfaction
- Maintain thorough and accurate medical records using electronic health record (EHR) systems
- Participate in team meetings, quality improvement initiatives, and professional development opportunities
- Dedicate administrative time included in your work schedule for charting and follow-up
- Ensure compliance with all clinical, regulatory, and ethical standards

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Valid and unrestricted medical license
- Commitment to value-based, patient-centered care for seniors
- Excellent clinical, communication, and organizational skills
- Ability to work collaboratively within a multidisciplinary team
