This role offers an opportunity to work in a collaborative team environment, supported by administrative and clinical staff, including a dedicated Medical Assistant, Scheduler, and Referral Specialist. The Primary Care Physician (MD/DO) will serve a small, manageable patient panel, ensuring quality time and personalized attention for each patient. The position is 100% outpatient, with no nights or weekends required, supporting an excellent work/life balance. Our clinics are equipped with in-house consulting specialists and holistic health services, including cardiology, podiatry, acupuncture, tai chi, yoga, and nutrition programs. Primary Care Physician (MD/DO) - Summary- Full-time Primary Care Physician (MD/DO) role focusing on value-based outpatient care for seniors

- Provide comprehensive primary care services to a panel of up to 450 senior patients, focusing on individuals with multiple chronic conditions
- Manage a schedule of approximately 18 patients per day, ensuring personalized, high-quality care
- Conduct patient assessments, develop and implement treatment plans, and manage ongoing care coordination
- Collaborate with in-house consulting specialists (e.g., Cardiology, Podiatry), holistic health practitioners, and support staff
- Utilize value-based care principles to improve patient outcomes and satisfaction
- Maintain thorough and accurate medical records using electronic health record (EHR) systems
- Participate in team meetings, quality improvement initiatives, and professional development opportunities
- Dedicate administrative time included in your work schedule for charting and follow-up
- Ensure compliance with all clinical, regulatory, and ethical standards

- Valid and unrestricted medical license
